for your next MJBT event. You can complete this form online, print it
out, and send with your entry fees to:

Just a reminder, entry forms/fees must be received by or on the Wednesday prior to the
tournament date, otherwise a $10.00 late fee will apply. To avoid
late fees, the entry should be postmarked at least 7 days in advance
of the deadline date. The MJBT shall not be held responsible for any
late, lost, or misdirected mail. |

The
$35 First Timer Entry fee applies to the first event a new player
ever competes in - it does not carry thru the complete season,
or following seasons. |

DO NOT MAIL ANYTHING
TO THE MJBT AFTER THE MONTHLY DEADLINE HAS PASSED. It will not be
received in time for the tournament. You will need to enter the
tournament on that specific day as a walk in entry (late fee will
apply) |

At this time, we
do not accept credit cards for payment. Mail in entries must be
paid by check or money order (in US funds). |

If you have won a
free entry - please specify on the form. Starting Aug 2007,
certificates for free entries will be given and will be required
to be sent with entry form. |
How to use this form...
- Fill in all of the fields below. Please
DOUBLE CHECK ALL INFORMATION IS CORRECT BEFORE NEXT STEP
- Press the SUBMIT button ONCE at the bottom
of the page.
- PRINT entry form from the Entry Confirmation screen
that appears. This should print a one page document for you to send
with the applicable fees to the MJBT.
- Use the RESET button to either start over
and/or create another entry.
NOTE - Some fields may not print correctly - we are working to
fix these issues as soon as possible. Please hand write any missing
field before sending with entry fees
